What happens when somebody simply don't do what he or she is responsible for?

I am trying to understand Holocracy, and I have rather basic question:

1. When somebody simply don't do what he or she is supposed to do, or when he or she do it in the quality or level different then expected by others - what happen then?

2. Another similar case: when you have few people in same role, i.e. in 3 shift operation, and some of them simply work slower and some faster, some with better quality some with lower - what then?

(I know it is very traditional thinking but I feel a need of manager in this situation... )

Can you explain?
